Who We Are

Kissito Healthcare Hondo is a 90 bed dually certified skilled nursing facility offering rehabilitative services and long term care for residents with moderate to complex medical conditions. The facility is located about 20 miles outside San Antonio, TX in Hondo, TX. Our facility serves Medina and Bexar counties and all parts in between.

We are part of Kissito Healthcare, a not-for-profit healthcare provider committed to growing through the continued development of our Collaborative Patient Care Pathway™. This patient-centered approach emphasizes collaboration, coordination and partnership among patients, families, hospital caregivers and post acute healthcare teams to produce smoother care transitions, improved outcomes and reduced costs.

Our Philosophy Of Care

Our unique Collaborative Patient Care Pathway™ treats each resident as an individual. Interdisciplinary teams comprised of — physicians, nurse practitioners, licensed nurses and therapists — work closely with residents and their families to develop personalized care plans based on their special needs, preferences and life experiences.

Each resident receives a care manager who meets with the resident one to three times a week, and with the resident’s family at least monthly, to assure consistent care and progress toward personal goals.

Rehabilitative Care

When patients are discharged from the hospital, our goal is to produce smoother care transitions and improved outcomes for the patient. We rigorously “track and trend” the progress of each resident to ensure the best care in a setting that includes:

• Private and semi-private rooms
• Rehabilitation gym with state-of-the-art equipment
• Physical, occupational, and speech therapy
• Innovative activities and programs

Long Term Care

For those who need a longer stay, we offer long term care services for medically complex residents, including those with the following needs:

• Wound Care
• Infection Control
• Post-Operative Care
• Pain Management

Our Healing Environment

We believe a warm, nurturing environment is essential to healing. To enhance our down-home atmosphere, residents are encouraged to decorate their own rooms, and families and friends enjoy unrestricted visiting hours.

Activities are structured around individual needs and include innovative programs designed to stimulate and engage each resident’s personal interests. Our culinary team provides delicious country cooking, adapted as required to therapeutic diets prescribed by physicians.
