Hondo’s Social Services primary function is to facilitate the admission and discharge process, making it as uncomplicated and positive as possible. Admissions require liaisoning with the referral source, insurance verification, meeting the potential resident’s family and providing tours of our facility. Social Services provide information regarding financial matters, resident’s rights, facility practices, advanced directives, and appointment of personal representatives. During the discharge process, the social workers responsibilities include arranging for any necessary equipment, home health services, and follow up appointments so our patients are well taken care of after they leave our facility.
The Director of Social Services, addresses all psychosocial needs the resident may present, including depression, anxiety, dementia and delirium, loss of relationships, loss of personal control and help our patients adjust to the facility. It is my responsibility to monitor these needs and ensure they are adequately met. The Social Worker is key in promoting and assuring continuous quality of life.
